Learn about Survivorship Care Plans – a communication tool between your oncology/haematology treating team, you, your GP and any other health professionals involved in your care.

Care plans are a written document with:

  • a summary of your treatment including your cancer diagnosis, the type of treatments received and side effects experienced
  • a follow up care plan, including follow up appointments, future tests and scans required, and possible late and long-term side effects
  • a wellness plan including recommendations for healthy living based on your individual needs.

The Australian Cancer Survivorship Centre has developed a Care Plan tool for 6 major types of cancer: early stage breast, early stage bowel, localised prostate, early stage melanoma, uterine (endometrial) and non-Hodgkin lymphoma.

Online proformas are available for people with other cancers who’re interested in creating their own care plans.

Presenting this session is Nicole Kinnane, Project Manager, The Australian Cancer Survivorship Centre, and a Nurse Consultant in the GynaeOncology Services at Peter MacCallum Cancer Centre. Nicole’s work has a strong focus on research that promotes cancer survivorship.
