    Thank you for your interest in Counterpart and your willingness to apply to be part of our volunteer program. Please complete the following information to help us gain a better understanding of you and your cancer experience.

  • Part 1 – Personal information



  • Part 2 – Volunteer training

  • The Peer Support Volunteer training will be conducted online at your own pace, and will include a compulsory* in-person induction day for all new volunteers.

    The training will also include one day of online workshops (also compulsory, on a Tuesday, date to be confirmed) as well as face-to-face “on-the-job” learning on a regular basis for those who can travel to the Melbourne CBD. Trainee volunteers from regional Victoria will have the option of partaking this “on-the-job” training online with experienced volunteers.

    *Counterpart will reimburse trainees for the cost of travel to Melbourne for the induction day.

  • Please note that attendance is required. If you can’t arrange to attend on this day, you may wish to apply for our next intake.


  • Part 3 – Availability


  • 22. On the successful completion of the training, which days will you be available to volunteer?

  • This is only an estimate. We understand this may vary over time due to other commitments.
    This is not a prerequisite for volunteering. Please only tick yes if you have a car and appropriate driver's licence.


  • Part 4 – Cancer experience

    To ensure our volunteer group is made up of women with a range of different cancer experiences, we would like to know more about your diagnoses and treatment.
  • Please provide details about type/s and month/s and year/s of your diagnosis.
  • Please specify month and year.
  • Please indicate if you have had additional surgeries as a result of your cancer eg: breast reconstruction, stoma, plastic surgery etc.


  • Part 5 – Other information


  • 35. Please provide the name, address and telephone number of a referee whom we can contact. This can be a relative, friend, work colleague or anyone else that you would be happy for us to speak to. Please explain their relationship to you.
